If you are aged 18 years or older and are applying for a Race, Rally or Kart licence from the Motor Sports Association you should obtain a competition licence application form. This medical costs £100 (including a free eye test) payable by cash or cheque (made payable to "Catherine Colvin").
What to bring:

Application form for Licence obtained from the Motor Sports Association

Photo ID (passport or driving licence)

A completed North East Driver Medical questionnaire which can be downloaded here

If you wear glasses bring them with you. Contact lens users should wear their glasses for the medical

A urine sample

A copy of a 12 lead ECG if applying for an International licence if under 45 years old. If older than 45 years a written report on a stress-related ECG is required.

Payment by cash or cheque (made payable to "Catherine Colvin") for £100. Receipt supplied
